Trusted Assessor Communications for Homes.

Care Homes have a duty to complete a pre assessment when a person requires
admission to a nursing/care home. This also may be a person returning to their
nursing/care home requiring a reassessment. This assessment ensures the care
home can meet that person’s needs.
Nationally there have been delays in discharge from hospital due to delays in this
assessment, and the Trusted Assessor model has been introduced on a national
scale as part of the High Impact Change Model to reduce Delayed Transfers of Care.
The Hampshire Care Association is running a Trusted Assessor scheme across the
Portsmouth and South East Hampshire area. The Trusted Assessor is based at
Portsmouth Hospital Trust (QAH) with the aim to facilitate timely and transparent pre
assessments on behalf of all home managers. The Trusted Assessor is here to
support all home managers and will undertake the pre assessments on behalf of the
care homes across South East Hampshire, Fareham, Gosport and Portsmouth.
This is a free service to all Care homes within this area.
